Registration and Logistics
This course is held at CHSC Shalom Retreat and Training Center in Grand Saline, TX. Course activities are scheduled from 12 noon Thursday, September 19, 2019 through 5:30 pm Saturday, September 21, 2019. When making transportation arrangements, please plan 2 hours driving time between Dallas airports and Shalom Retreat and Training Center. Participants are invited to stay at Shalom Retreat and Training Center and encouraged to arrive September 18 and depart September 22.
If you must cancel your registration, CHSC will refund the full amount paid minus an amount equal to the course deposit ($50).
If CHSC cancels a course (for example, due to low enrollment), we will inform all registered participants 1 month prior to the course start date and will refund the full amount paid. Participants are solely responsible for travel plans and expenses. We encourage registrants to make travel plans after the 1 month course confirmation date.

nonCMDA members – Workshop tuition $150; An additional post-activity CME credit fee of $10/credit will be assessed – instructions for payment will be provided at the workshop.
